District Convention Nominations Part 2: Candidate Profile

Candidates who have been recommended for  and agree to stand for nomination should fill in the form below.

According to the Nebraska District Bylaws, “background information on all persons who have been nominated and agree to run,” must be distributed to all voting delegates of convention. The information from your form will be used to prepare a written profile for delegates to review prior to voting for the position to which you have been nominated. Thank you for your cooperation. Please assist the nominations committee by filling out your candidate profile form as soon as possible.

After you submit your candidate profile:

All candidates are asked to provide a headshot for use in the Convention Workbook. The file can be in black/white or color, and sent as a .png or .jpeg file. The file should be titled with your first and last name. (Example: john_smith.jpg)  Please send your photo to candim@ndlcms.org as soon as possible.
